San Bernardino County Department of Behavioral Health (DBH):

The San Bernardino County DBH Prescription Drug Program provides payment assistance for certain behavioral health-related prescription medication to San Bernardino County residents who have met program eligibility requirements.

Members who have been determined eligible to receive prescription drug payment assistance from the San Bernardino County DBH will receive a client identification card from the County. They can present this card to a contracting pharmacy to get their medications.

What is the Prior Authorization Process?

Prior authorization is required for all drugs that are not listed on the formulary and for prescriptions that fall outside of the dispensing guidelines as indicated on the formulary.

The pharmacy or medical provider must complete the prior authorization form and fax it to the San Bernardino Department of Health at 1-909-382-3106. Please call 1-909-382-3087 for questions about the prior authorization approval process.
